Important Notes:
1. Click the session below that you would like to apply for CE for.
2. You must fill out one form for each session that you would like to receive CE for.
3. Please answer all questions thoroughly. The purpose of the survey is to prove that you watched and understand the content in each session and credit can only be awarded to those providing enough information to prove attendance.
4. Once we receive your submission, we will verify your ticket purchase prior to awarding credit.
5. It may take up to 45 days for your CE to fully credit process.
